COVID Isolation & Quarantine Documents, Assistance and Guidance
New York State has published an all-inclusive website for COVID assistance, "NYS's Approach to Isolation and Quarantine: What to do if you test positive, were exposed to someone who tested positive or display COVID‑19 symptoms." This site provides links for official isolation and quarantine documentation, as well as access to NYS isolation & quarantine guidance documents and information on what to do if you test positive, were exposed or have symptoms of COVID. Additional resources for schools, employers and general facts & questions are also available. All Herkimer County residents seeking isolation and quarantine documentation should selected the respective form or click here to find on the NYS website.
All individuals ages 5 and up are eligble for vaccination. For more information, please call our office at 315‑867‑1176.
Herkimer County Office for the Aging can assist individuals 60 and older with scheduling a COVID‑19 vaccination. You will be put on a waiting list and you will be called when appointments become available. Call 315‑867‑1121 or 315‑867‑1371.
Use Vaccines.gov to find available vaccines within 25 miles of your location. Just enter your zip code and a list of locations will be displayed.
