Herkimer County Office for the Aging
Herkimer County Office for the Aging/NY Connects is assessing the needs of older adults in our community. Every four years our office completes a Needs Assessment to determine what types of services older adults in our community need to remain living independently in their homes.
Part of the Needs Assessment is a survey that we are asking older adults and any service providers who work with older adults to complete. Once the Needs Assessment is completed we will utilize the information to plan services for the next four years. We need your help.
