According to Medicare discharge planning is "A process used to decide what a patient needs for a smooth move from one level of care to another."

Did you know that studies have shown that as many as 40 percent of patients over 65 had medication errors after leaving the hospital, and 18 percent of Medicare patients discharged from hospital to home are readmitted within 30 days. On the other hand, research has shown that excellent discharge planning and follow-up can improve a patients' health, reduce re-admissions to the hospital, and decrease healthcare costs.
